Great Chapel St Medical Centre is dedicated to enhancing the health and wellbeing of individuals facing homelessness, insecure housing, social isolation, or poor health.
The Practice is run by the Homeless Health CIC.
Our vision revolves around promoting excellence in holistic medical care and support for this vulnerable population. We strive to break down barriers to access and foster patient- centred services, aiming to reduce health inequalities through education, research and advocacy.
With a focus on complex patient management, we prioritise individuals who struggle to receive adequate care, ensuring they receive the attention and support they need to improve their health outcomes and overall quality of life. We design services around the needs of our patients.
Our hybrid nature allows us to combine medical care and social care, going beyond the very essential interventions in medical care. For example, we can escort patients to services for specialist interventions; we facilitate them with clothing or a shower. We are a very flexible service adapted to a very complex client group Our staff are committed to providing quality healthcare for the benefit of all our patients.
We are a part of Safe Surgeries and committed to tackling the barriers to healthcare faced by migrants.

About the PCN
Our Practice is part of the West End and Marylebone Primary Care Network (PCN)
There are 1,250 PCNs across England based on GP registered patient lists, typically serving natural communities of between 30,000 to 50,000 people. PCNs bring together GP Practices, community, mental health, social care, pharmacy, hospital and voluntary services. West End and Marylebone PCN provides a range of innovative services and pilot projects including Out of Hospital and extended hours services.

Great Chapel Street Medical Central is part of Healthcare Central London. This GP Federation covers the whole of Westminster and includes the West End and Marylebone PCN. HCL supports us to provide services for our residents including out-of-hours appointments and cardiology and diabetes hubs.
